Admission Criteria

• Living in the hospice service area
• Life expectancy of six months or less
• Approval of patient's attending physician
• Patient and family accepting palliative care

• Family Home
• Respite Facility
• In–patient Skilled Nursing Facility
• In–patient Hospital Facility
• Continuity of care between Locations
